Our legal responsibilities
We are required by law to give you this notice. It describes how we may use and disclose protected health information about you and your rights. We shall maintain the privacy of protected health information and provide you with notice of our legal duties and privacy practices. We may change these policies at any time and will notify you of changes. You may request a copy of our notice any time by contacting us at the address below.
How we may use or disclose your protected health information
Treatment: We may use and disclose your PHI to provide treatment via telehealth (phone call or video), including sharing with physicians and staff involved in your care.
Payment: We may use or disclose your PHI to obtain payment for services you receive, including billing, payment processing, and related administrative activities permitted by law. We have written agreements with any payment or billing vendors to protect your PHI.
Health care operations: We may use or disclose your PHI to operate the practice (quality improvement, training, appointment reminders). We have written contracts with business associates (e.g., electronic health record, hosting) that protect your PHI.
We will not use or disclose your PHI for any other purpose without your specific, written authorization. You may revoke an authorization in writing at any time.
Intake and telehealth: We collect your symptoms and history through a structured visit intake before your visit. A licensed physician reviews all information and makes all decisions. Your intake and documentation are shared with your physician and stored in our electronic health record. Care is provided via phone call or video; we use vendors that meet privacy and security standards.
We may also disclose your PHI as required by law, for public health, health oversight, judicial process, law enforcement, workers’ compensation, and other purposes permitted by federal and state law.
Your rights
- Access to medical records (written request; fee may apply)
- Amendment if you believe PHI is incorrect or incomplete (written request with reason)
- Accounting of disclosures (written request; limited lookback per law; fee may apply)
- Restriction on use/disclosure except where required by law (written request)
- Confidential communications in a certain way or location if reasonable
- Paper copy of this notice if you signed electronically
- Complaints to our office or the U.S. Department of Health and Human Services; we will provide the HHS complaint filing address upon request. We will not retaliate against you for filing a complaint.
